Coaching Education Reimbursement Form

Coaches will need to submit this for PRIOR to signing up for a coaching education course

Coaching Education Reimbursement Protocol(Required)

I agree to the Coaching Education Reimbursement Protocol (below)

The following policy applies to all coaches and directors of Premier Futbol Academy who wish to take a coaching course or continuous education course.

The following protocol must be adhered to by the coach seeking reimbursement:

– Must present a complete application to the club’s Director of Coaching (DOC) & Executive Director (ED)at least 7 days in advance of registering for the course
– Register and pay for the course
– Must attend the course and successfully complete the course with a passing grade to be considered for reimbursement
– Submit receipts for all expenses incurred
– Submit a certificate as proof of successful completion to DOC & ED
– Disclose any other financial assistance granted to them for the course
– Must have Remained with Premier FA to be eligible for reimbursement

Coaching Education Reimbursement Policy(Required)

Premier FA will issue reimbursement as follows:

– Grassroots Courses:
Full Course fee immediately. Only the cost of the course, no other costs will be reimbursed.
-US-D License:
Only the cost of the course, no other costs will be reimbursed.
1st Half – Immediately upon submission of certificate
2nd Half – One (1) year after passage of course
– A-B-C- National Youth License:
Reimbursement will include lodging and meals that are not included with the course registration fees. Only approved travel expenses (airfare, or 38 cents/mile).
1st Third- immediately upon submission of certificate
2nd Third – One (1) year after passage of course
3rd Third – Two (2) years after passage of course

Premier FA Discretion Policy(Required)

I agree to Premier FA’s Discretionary Policy

Reimbursement approval will be made at the discretion of Premier FA. Approval will be made prior to registration for the course. This ensures that the club stays within budget while supporting the educational needs of the coaches within the club.
